Charity Care provides discounted services or relief of debt incurred at NCCH for Arizona residents who are eligible under Arizona poverty guidelines. All applications approved or denied will remain on file for 6 years and will be available for audit and cost reporting purposes.
The purpose of the Charity Care policy is to outline the policy and procedure upon the identification of patient financial hardship and thereof providing guidance to Patient Financial Services personnel in determining eligibility per poverty guidelines.
Procedure:
I. Financial Hardship is identified; patient is given Charity Care Application.
II. Application is completed and all required documents are obtained.
III. Application and documents are prepared with a cover letter attached to the application to begin process for approval/denial.
IV. Completed Applications are submitted to the Patient Financial Director for approval/denial. a. If additional information is needed to complete approval/denial process, must obtain and resubmit completed application. b. If application is denied it will be marked accordingly and all paperwork will be filed
V. Upon PFS Director’s approval the completed application is submitted to CEO/CFO for approval/denial. a. If the request is denied the completed application is marked accordingly and all paperwork will be filed. Patient is sent a denial letter.
VI. Upon CEO/CFO approval the completed application is marked accordingly and all paperwork will be filed. Patient is sent an approval letter. Approval letter is also submitted to Data for adjustment(s) to be posted on account(s) covered under the Charity Care Application
Northern Cochise Community Hospital offers financial assistance for medical care to eligible patients. Our financial assistance program includes discounted services or relief of debt incurred at NCCH Inc. You may be eligible for financial assistance if you:
- Are a resident of Arizona Are not eligible with AHCCCS
- Are uninsured and/or have limited health coverage
- Can show financial hardship
- Can provide necessary information and documents about your household finances
Application Process:
- Complete the Financial Assistance Application
- Go over Financial Assistance Application with Financial Counselor
- Completed Application is submitted to Patient Financial Services Director to be approved or denied and following to CEO or CFO to be approved or denied
- We contact you via phone and by mail
- If your case has a remaining balance, we will setup a payment agreement
