Swing Bed

Swing Bed is around-the-clock skilled nursing and hospital-based physician care plus the benefit of rehabilitation therapies, as needed, to help patients return to their previous living environment. From planned joint replacements to those recovering from a recent stroke, our Swing Bed services help improve patients' level of capability, independence and function.The Swing Bed program is designed to serve individuals who are no longer in need of acute hospital care but, instead, are in a transitional phase of recovery from an illness, injury or surgery. The service allows patients the time to heal and adjust to everyday life by providing robust therapy up to twice each day, Monday through Friday. A collaborative team approach is used to facilitate efforts of your physician, nursing team, therapists and case manager.Whether you are admitted directly to NCCH or transferred from another hospital, you can depend on us to work with you, your physicians and health care team to deliver the care you need.

Case management

Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality cost-effective outcomes.Our case managers work and communicate with your insurance company, while keeping you informed of your rehabilitation goals and your plan for discharge. Your patient care team meets weekly to make recommendations for your continued progress and coordinate discharge planning with your doctor and family.Your family is important to your recovery and we encourage family members to participate in these meetings at your request.
